Performance evaluation forms allow an organization to define the criteria that team leaders use to evaluate the performance of their team members. The forms are built from sections and questions, and you can define a custom rating scale and question-specific weightings for them. Administrators maintain these forms centrally so that team leaders always have access to up-to-date and consistent evaluation criteria.
Where to Find Performance Evaluation Forms
Performance evaluation forms can be managed under Administration > Surveys in the Performance Evaluation section. On this page, you’ll see a list of all forms created for your organization under Available Forms.
Please note that performance evaluation is a feature that must be enabled on a per-customer basis. If you do not see the menu, contact your administrator or support to enable this feature.
Creating a New Form
- Go to the performance evaluation forms view and select Add New Form.
- Enter a Name for the form and, if necessary, a Description to help team leaders understand the form’s purpose.
- Add sections to the form by selecting Add New Section and enter a title for the section.
- Add questions to each section by selecting Add New Question. Each question is given a title and, if necessary, a weight (Low, Normal, or High), which affects how much the question contributes to the section’s average.
- Select whether the weighting is also displayed on the form to the team leader conducting the evaluation using the setting Show Weighting on Form.
- Define the form’s Rating Scale and descriptions for the different levels of the scale, such as “Below Expectations,” “Meets Expectations,” and “Exceeds Expectations.”
- Save the form.
Editing and Duplicating a Form
You can open an existing form for editing from the list. In the editing view, you can add, delete, and reorder sections and questions, as well as update the rating scale.
If you want to quickly create a new form based on an existing one, use the Duplicate function. This is useful, for example, when you want slightly different evaluation criteria for different roles or teams based on the same basic structure.
Activating the Form
In order for the form to be selectable by team leaders when starting a new evaluation, it must be set to Active. Only active forms can be selected as the basis for evaluating a new performance, so be sure to activate the form once it is complete.
If you no longer want the form to be used in new evaluations, you can mark it as inactive without losing any previous evaluations conducted using that form.
Deleting a Form
You can delete a form from the list view by selecting Delete. Please note that deletion is permanent, so make sure the form is no longer needed before deleting it.
How to Use Forms
Once a form has been created and activated, team leaders will see it as an option when they start a new performance evaluation for a team member from the team leader’s view. Completed evaluations appear in both the team leader’s and the employee’s personal development summary, and their results can also be viewed in summary form in the Performance Evaluations reporting view.
